Labs & Consent Forms | Hip

You will be asked to sign the following consent forms to show that you have been given and understand the information you need to decide to have surgery. We want you to be informed before you sign these forms. If you have any questions, please ask.

Consent for Hip Surgery - This is for the surgeon's office.

Informed Surgical Consent - This is for the hospital record.

Informed Consent for Blood Product Transfusion - This is your consent for you to be given a blood transfusion should one become necessary.

Authorization of Medical Care - This is general permission to care for you while you are a patient in the hospital.

Patient Consent Form - This is needed if you donate your bone to the bone bank. Before the bone can be used we must show proof that the bone is healthy, so we will need to test your blood at your six-month evaluation. There is no charge for this test, but we do need your permission.

Special Consent - If you take part in a research study, the project will be explained to you and you will be asked to sign a consent form for your participation.

Anesthesia Consents, A & B - These are consents for you to receive anesthesia during surgery.

Photo Consents and Release of Information - There are occasions when a patient may be asked for his or her permission to be photographed during a procedure or while at the hospital, or for permission to be interviewed by a member of the media. These opportunities are generally to help educate and inform others about the procedure. The Release of Information and Photo form will contain a description of where the photo or interview information will be used.
